Job Role: Deputy Manager - Discharge to Assess (Reablement) Team

Job Type: Permanent, Full time 37.5 hours per week.

Location: Glastonbury, Head Office

A rare opportunity has presented itself, as Candlelight Care are now looking for a Deputy Manager t o provide support to the D2A Registered Manager (RM) by assisting to run the day-to-day activities of Candlelight\'s D2A Service. You would be expected to work in a way that complies with all relevant legislation, incorporating internal policies and procedures, and be guided by NHS processes and it\'s expectations. You will also play an important role in supporting the D2A RM, by deputising in their absence and undertake the HR/Payroll related paperwork, which needs to be submitted in a timely manner to Payroll.

We are looking for someone who is passionate, dedicated and has the motivation to make a positive difference to the service, and the company as a whole. This role would suit someone who may already be a successful deputy manager with a strong track record and knows their community well. Deputy Manager Responsibilities:

The position requires leadership ability and good people skills, the ability to communicate well and the knowledge and experience to take initiative.

  • To regularly review capacity within the service and ensure the regular and effective flow of patients from hospital, back into the community.
  • To ensure all patient details are entered accurately and in full, into the Access System, and to ensure patient visits are allocated and staff availability is used most effectively.
  • To manage the needs of patients daily, ensuring visits are reduced and ceased in a timely manner. To monitor patient progress daily, focussing on the D2A goal achievements, and to regularly liaise with the Patient Liaison Officer to ensure a high quality and effective service is delivered.
  • To ensure patient care plans are written in a person-centred way and comply with company policy.
  • To support the Registered Manager to monitor staff recruitment, induction training as well as support sessions, supervisions and community observations. To have an active involvement with training and development, to ensure the D2A can meet the changing needs of patients.
  • Ensure risk assessments are carried out and reviewed regularly.
  • To monitor the NHS RIO system, ensuring staff are uploading notes as per the NHS guidance.
  • In the absence of the Registered Manager, to attend all operational meetings, representing the company in a knowledgeable and professional manner. To attend face-to-face and cluster calls via Microsoft Teams as needed.
  • To maintain an excellent level of communication between Candlelight, the NHS keyworkers and all other professionals involved in the care and support of D2A patients.
  • To promote a patient friendly atmosphere which strengthens and develops links with colleagues and the NHS to maximise cooperation to benefit both the Company and the patient.
  • Develop a culture which promotes and continually improves a safe working environment for the staff, team and patients, in accordance with National Health & Safety legislation.

Requirements:

  • Experience of working in the care industry, with knowledge of Reablement.
  • Drivers Licence and transport.
  • An enthusiastic personality and strong leadership skills.
  • Experience in leading a team to meet goals.
  • Experience of working to tight deadlines.
  • NVQ level 4 in Health and Social Care, or equivalent, or a willingness to work towards this.
  • A passion for providing quality care.
  • Be hardworking and dedicated to your role.
