We are looking for an experienced Nurse Practitioner/Paramedic Practitioner/Physiotherapist to work within ESNEFT's Urgent Treatment service in Clacton, providing a holistic approach to the care of individuals. The UTC is open 7 days a week with practitioners expected to work across all days.
We are looking for an experienced nurse or allied health professional who is proficient in assessment, diagnosis and treatment of minor injury, minor illness and urgent presentations as appropriate, working within the scope of professional practice and agreed guidelines and protocols. This post will cover the UTC at Clacton and Minor Injuries Unit at Harwich.
You will work with colleagues across health, social care, mental health and the voluntary sector to ensure holistic and joined up delivery of care. You will empower patients to make decisions about their own care and encourage them to exercise their rights and choices, signposting them to relevant community and voluntary services and supporting patients, carers and members of the integrated team with management of urgent presentations.
To utilise clinical expertise, function as an experienced clinician managing the assessment, diagnosis and treatment for patients with a range of urgent care presentations. To have clinical autonomy regarding the clinical diagnosis and future clinical management of presenting patients through clinical assessment and the requesting and interpretation of the results of specific medical investigations.
Support patients with advice, information, persuasion and direction to community resources to encourage self-management and ongoing independence. Provide support, training and advice to patients, parents and carers. To have the ability to make clear decisions with confidence and communicate these effectively. To maintain appropriate clinical records in line with current practice guidelines.
