Job Description
We are looking for an experienced and compassionate Senior AACHC Nurse Assessor to join our Continuing Healthcare team. You will play a vital role in assessing, reviewing, and case-managing adults with complex health needs, ensuring they receive the right care, in the right place, at the right time.
This is a varied role where you will complete comprehensive CHC and Funded Nursing Care assessments, lead on case reviews, participate in appeals, and work collaboratively with patients, families, carers, and multi-disciplinary professionals.
Key Responsibilities
- Undertake and lead Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments in line with the National Framework.
- Act as Case Manager for individuals in receipt of CHC funding, ensuring care is safe, appropriate, and person-centred.
- Facilitate and chair multi-disciplinary team (MDT) meetings and present cases at CHC review panels.
- Support patients and families through the CHC process with empathy, clear communication, and advocacy.
- Monitor, review and evaluate packages of care to achieve best outcomes.
- Work closely with local authorities, NHS teams, care providers and partner organisations to ensure seamless care.
- Uphold safeguarding responsibilities, equality, and diversity in every aspect of care.
About You
We are seeking a Registered Nurse (NMC):
- Experience of assessments and case management for people with complex, long-term conditions, disabilities, or palliative needs.
- Strong understanding of the National CHC Framework, FNC, and related legislation.
- Excellent communication, negotiation, and conflict resolution skills with the ability to manage sensitive discussions.
- Ability to work autonomously, prioritise a busy caseload, and contribute to service development.
- Commitment to patient-centred care, safeguarding, and tackling health inequalities.
Desirable: leadership experience, audit/quality improvement, knowledge of commissioning and financial processes.
