About the Role
We are seeking an experienced Band 6 Community Psychiatric Nurse to join our Community Mental Health Recovery Service in Frimley. This is an excellent opportunity to work within a dynamic multidisciplinary team, supporting individuals with mental health needs in the community.
Key Responsibilities
- Undertake comprehensive initial assessments of individuals accessing mental health services
- Support the management and allocation of waiting lists
- Conduct thorough mental health assessments, formulating both mental health and social needs of service users
- Make evidence-based recommendations for onward care, including external referrals
- Present case formulations and care recommendations within MDT meetings
- Develop person-centered care plans with robust risk assessment and management strategies
- Liaise effectively with external agencies and key stakeholders to ensure high-quality, coordinated care
- Engage with service users' support networks, including carers and family members
- Deliver brief interventions such as emotional coping skills, trauma stabilization, and psychoeducation
Essential Requirements
- Registered Mental Health Nurse (RMN) with valid NMC registration
- Relevant NHS experience in a community mental health setting
- Excellent verbal and written communication skills
- Proven ability to conduct comprehensive mental health assessments
- Strong care planning and risk management capabilities
- Experience working collaboratively within multidisciplinary teams
Desirable
- Experience using SystmOne clinical system
- Skills in delivering brief psychological interventions (training can be provided)
