About the Role

We are seeking an experienced Band 6 Community Psychiatric Nurse to join our Community Mental Health Recovery Service in Frimley. This is an excellent opportunity to work within a dynamic multidisciplinary team, supporting individuals with mental health needs in the community.

Key Responsibilities

  • Undertake comprehensive initial assessments of individuals accessing mental health services
  • Support the management and allocation of waiting lists
  • Conduct thorough mental health assessments, formulating both mental health and social needs of service users
  • Make evidence-based recommendations for onward care, including external referrals
  • Present case formulations and care recommendations within MDT meetings
  • Develop person-centered care plans with robust risk assessment and management strategies
  • Liaise effectively with external agencies and key stakeholders to ensure high-quality, coordinated care
  • Engage with service users' support networks, including carers and family members
  • Deliver brief interventions such as emotional coping skills, trauma stabilization, and psychoeducation

Essential Requirements

  • Registered Mental Health Nurse (RMN) with valid NMC registration
  • Relevant NHS experience in a community mental health setting
  • Excellent verbal and written communication skills
  • Proven ability to conduct comprehensive mental health assessments
  • Strong care planning and risk management capabilities
  • Experience working collaboratively within multidisciplinary teams

Desirable

  • Experience using SystmOne clinical system
  • Skills in delivering brief psychological interventions (training can be provided)
