Main duties of the job:
- Providing personal care, wound care, and prescribed equipment.
- Working alongside colleagues to respond to service needs.
- Good communication and collaboration as part of the Urgent Community Response nursing and therapy team is essential.
- Regular supervision and professional development opportunities.
- Focus on admission avoidance to secondary care and supporting discharges from secondary care or in-patient units.
Detailed job description and main responsibilities:
- Contribute to the delivery of care pathways for patients with long term conditions, palliative care, and rehabilitation needs.
- Work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
- Support early discharge from hospital.
- Provide a seamless, integrated service to our service users.
Person specification:
Qualifications:- NVQ Level 3, or equivalent knowledge and experience.
- Good general education, including GCSE Grade C in English and Maths.
- Additional competencies in wound care.
- Experience of working within a health or social care setting.
- Able to communicate effectively using the English language in both verbal and written form.
- Good interpersonal skills.
- Able to work as part of a team.
- Highly motivated and able to provide high standards of care.
- Tact and diplomacy, empathy and sensitivity.
- Ability to use own initiative and escalate to senior staff when necessary.
- Good observational and reporting skills.
- Ability to manage stressful situations.
- Experience of working within a community care setting.
- Knowledge of SystmOne and experience of end of life care.
- Professional appearance.
- The ability to kneel, bend & stoop, and work in cramped environments.
- Be reliable and aware of UCR service.
- Experience of working within a MDT.
- Must hold a full and valid driving licence and have access to a vehicle.
- Be able to make own travel arrangements to patients, clinics, base, and meetings as required.
- This is a 7 day a week service and some weekend working will be expected.
