We are currently looking for a dynamic and enthusiastic Occupational Therapist to join our Intermediate Care Team in Peterborough. The post holder will work as part of the multi-disciplinary team, and be responsible for ensuring that patients get the best care possible in their own homes, maximising potential to regain independence in our short stay service. Successful applicants will be expected to assist in the development of the integrated care workers to support the clinical delivery of the service and proactively work alongside the wider multidisciplinary team to ensure patient flow and safety.
Main duties of the job:- To work as an autonomous practitioner within the integrated multi-professional team.
- To be responsible for patients on the shared caseloads.
- To undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
- To identify patient needs, agree goals and provide appropriate therapy interventions, provide equipment and rehabilitation programmes.
- To take a holistic approach to all therapeutic interventions, working across professional boundaries at times.
- To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
- To provide support and education to peers, junior staff, non-registered staff, and students.
- To supervise and support education of junior staff and students, overseeing patient intervention and ensure that Band 5 Therapists, Therapy Assistant Practitioners, and Integrated Care Workers deliver high quality patient care.
- To be accountable for a delegated task and activities undertaken by Band 5 Therapists, Therapy Assistant Practitioners, and Integrated Support Workers.
- To prioritise all patients according to clinical need and to allocate or signpost tasks appropriately.
- To provide resilience and cover to other locality teams as required due to operational demands and system pressures.

Person specification:- Relevant post-graduate clinical experience in a variety of settings.
- Experience of equipment provision.
- In-depth knowledge of holistic assessment, problem-list creation, and goal setting.
- Community experience.
- Advanced moving and handling skills.
- Experience of SystmOne.
- Degree or equivalent qualification in Occupational Therapy.
- Documented evidence of own continuing professional development.
- Current HCPC registration.
- Member of RCOT.
- Time Management and prioritisation skills.
- Able to demonstrate commitment to high quality care and service provision.
- Ability to work flexibly as part of a team.
Ability to travel independently around Cambridgeshire and Peterborough area meeting required timescales. A Full driving licence and access to a vehicle is essential.
