We are currently recruiting for a Lead Reablement Worker based in Fenland. The Reablement Service provides care at home to people aged 18+ so that they can maximise their independence. The Lead Reablement Worker plays a vital role in supporting individuals as they transition towards independent living.

Main Duties

  • Act as the first point of contact for service users, working with Occupational Therapy teams and other health professionals to set and monitor personalised goals.
  • Develop tailored reablement plans that focus on recovery and independence.
  • Assist service users in relearning daily living skills (personal care, mobility, meal preparation, etc.).
  • Monitor progress, review plans and adapt support as required.
  • Collaborate with health and social care professionals, families and agencies to provide a holistic approach.
  • Maintain accurate records and documentation to ensure compliance and quality standards.
  • Refer users to community support services and monitor care throughout the reablement process.
  • Manage a varied caseload, ensuring timely, high‑quality support.

Qualifications & Experience

  • NVQ Level 3 or equivalent apprenticeship (or equivalent experience).
  • Proficient written and spoken English for case recording.
  • Strong communication skills; experience supervising support workers.
  • Knowledge of The Care Act, CQC requirements, and equality, diversity and inclusion principles.
  • Excellent verbal and written communication and IT skills.
  • Valid full driving licence with business insurance and access to a car for travel across Cambridgeshire.
  • Experience supporting individuals with care and support needs.

Training & Supervision

  • Receive excellent training and supervision as part of an established and supportive team.
  • A car‑lease scheme and weekend enhancement pay are provided.

Working Hours & Schedule

  • Weekly hours: 37, rota between 8 am and 4:30 pm, Monday‑Friday, with alternate weekends.
