The Cambridgeshire Learning Disability Partnership (LDP) provides an integrated health and social care service for adults with Learning Disabilities living in Cambridgeshire.

The LDP aims to ensure the assessment and provision of person-centred specialist health and social care services that promote an individual\’s right to choice, independence, health, and wellbeing, in line with their assessed needs, enabling them to reach their full potential.

We have an exciting opportunity to recruit 3 enthusiastic Band 5 Community Learning Disability Nurses to work across Cambridgeshire.

The posts are based in:

• Cambridge South, with the base in Papworth
• Fenland Team, based in March
• Cambridgeshire and East team, based in Ely

You will have relevant experience supporting people with learning disabilities in a community setting. As an autonomous practitioner, you will manage a caseload, providing assessments and interventions related to the health needs of adults with learning disabilities.

Responsibilities include:

• Participating in the delivery of specialist Learning Disability nursing services under the guidance of a senior nurse.
• Working with identified service users to provide assessment, identification, and treatment of their health needs.

Key responsibilities include:

• Demonstrating knowledge of working with adults with learning disabilities and willingness to undertake post-registration training.
• Understanding health and social care policies and legislation relevant to the service user group.
• Prioritising and managing a caseload with senior support.
• Assessing, identifying, treating, and managing health and nursing needs, including complex conditions and risk assessments.
• Providing advice and support to service users, carers, and partner agencies.
• Communicating effectively with multidisciplinary teams and external agencies, offering specialist LD advice.
• Implementing and evaluating care programs supporting individuals with complex needs.
• Understanding the specific needs during crises, such as mental health or physical health relapses.
• Recognising communication needs and tailoring interactions accordingly.
• Working in various community settings, including lone working and home visits.
• Preparing professional reports and action plans in line with guidelines and policies.
• Providing nursing advice to professionals within the LDP.
• Working to reduce health inequalities through partnership with primary and secondary care providers.
• Responding to local health needs and government initiatives.
