We have an exciting opportunity for a rehabilitation-focused Senior Occupational Therapist to join our Intermediate Care Service Team. The post holder will be required to work across both our North locality teams covering Huntingdon and Peterborough. The base for the post can be either Kingfisher House, Huntingdon or Phorpres Close, Peterborough.

This position involves working with people in their own homes and private provider care homes, supporting patients out of hospital and preventing admission. Applicants will be offering holistic clinical assessment, treatment, discharge planning, and specialist therapy knowledge. Successful applicants will play a vital role in supporting patient flow through the pathways, as well as providing clinical skills and knowledge to the MDT. This position offers flexibility, variety, and the opportunity to be involved in the transformation of our intermediate care services.

Main duties of the job
  • To work across Pathway 1 (care at home) and Pathway 2 (Health Interim beds) for the Intermediate Care service.
  • To work as an autonomous practitioner within the integrated multi-professional team.
  • To undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
  • To identify patient needs, agree goals, and provide appropriate therapy interventions, equipment, and rehabilitation programmes.
  • To take a holistic approach to all therapeutic interventions, working across professional boundaries at times.
  • To keep the patient at the centre of care, ensuring patient and carer participation in decision-making.
  • To provide support and education to peers, junior staff, non-registered staff, and students.

Detailed job description and main responsibilities
  • To undertake holistic assessment (including those with complex presentations and multi pathologies) making use of specialist clinical reasoning skills.
  • To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
  • To actively manage positive relationships with other agencies across the Cambridgeshire and Peterborough Integrated Care System involved in the patients' pathway.
  • To provide specialist advice to all members of the multi-disciplinary team, service users, and appropriate others regarding changes involving current care plans, patient progress, and other relevant matters that pertain to the care of the patient.
  • To support the day-to-day planning and delivery of high-quality care.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving license and have use of a car. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Person specification
  • Education/Qualifications: Degree or equivalent e.g., BSc, MSc, or Diploma entry-level qualification into the Occupational Therapy profession.
  • Current HCPC registration.
  • Documented evidence of own continuing professional development.
  • Member of RCOT.
Experience
  • Relevant post-graduate clinical experience in a variety of settings.
  • In-depth knowledge of holistic assessment, problem-list creation, and goal setting.
  • Experience of equipment provision.
  • Experience of completing moving and handling assessments.
  • Experience of using outcome measures and data to inform on patient outcomes.
  • Experience of SystmOne.
