The Home First Intermediate Care Service helps patients regain independence through early hospital discharge and avoiding hospital admissions. You will assist in delivering high-quality healthcare and support patients at home by following prescribed care and therapy plans.
The service\’s vision is to provide outstanding, patient-focused, community-based healthcare and therapy, supporting early discharge, assessment, and prevention of hospital admissions.
Operating hours are from 7 am to 10 pm, seven days a week.
Please note this position may be offered as a Band 2 Development post, depending on qualifications and experience. Full support will be provided to develop into a Band 3.
Main duties of the job
Travel is required around the county with strict time deadlines. A full UK driving license and access to a vehicle are essential. Public transport is not considered reliable for this role. Please confirm in your application that you meet these criteria.
- With training, apply skills from nursing, allied health professionals, and medicines management to delegated tasks.
- Perform competency-based treatment techniques under supervision, following prescribed plans.
- Provide care to patients experiencing health deterioration, focusing on avoiding hospital admission.
- Deliver functional assessments, personal care, medication prompts, mobility support, and equipment provision.
- Carry out duties delegated and supervised by registered professionals.
- Maintain high standards of care.
- Manage your own caseload of patients with complex needs requiring ongoing support.

- Assess patients’ responses to treatment plans and report findings to qualified professionals.
- Overcome barriers such as hearing or cognitive impairments.

Person specification
Education / Qualifications
- NVQ Level 2, Care Certificate, or equivalent experience.
- Willingness to undertake further training to meet competency requirements.
Experience
- Experience working in healthcare or care environments with older adults.
- Experience caring for adults and older people with physical or mental health conditions.
Skills & Abilities
- Commitment to high-quality care and service.
- Ability to record information accurately and concisely.
- Flexibility to meet patient and service needs.
- Experience using SystmOne or similar systems.
