The post holder will join an expert team of registered professionals within the wider Intermediate Care Service. The role involves clinical triage of referrals into CPFT for discharge pathways 1 (care and therapy at home) and 2 (health interim beds in private provider care homes), as well as managing patients in pathway 2.
The postholder should have clinical experience with patients following orthopaedic surgery and with cognitive impairment/delirium/dementia. Excellent communication skills are essential, as the role requires liaising with colleagues in Intermediate Care, internal teams, private care homes, care agencies, and system partners such as social care and Continuing Health Care teams. The role involves both remote work via Microsoft Teams and face-to-face assessments, reviews, and interventions.
The position covers Cambridgeshire and Peterborough, with bases in Cambourne, Ely, Huntingdon, or Peterborough. The main base is Huntingdon, but flexible location options are available across the county.
Please note: The role requires independent travel around the county to meet strict deadlines. Candidates must hold a full UK driving licence and have access to a vehicle. Public transport is not suitable for this role. Please confirm in your application that you meet these criteria.
The role involves acting as a specialist registered professional providing assessment, rehabilitation, and discharge planning support for complex patients transitioning from hospital to community settings. You will manage flow into health interim beds and deliver expert assessments and discharge planning for patients on our pathways across Cambridgeshire.
