We are looking for an Associate Practitioner to work within the Learning Disability Partnership service, within the South LDP Team. The Cambridgeshire Learning Disability Partnership (LDP) consists of six integrated specialist health and social care teams as well as local authority run day and accommodation services for adults with a Learning Disability.

This is an Ideal opportunity for people who wish to develop a wide variety of skills in working with adults who have a learning disability. The successful application would be working within the MDT alongside arts therapists, nurses, occupational therapists, physiotherapists and speech and language therapists.

The role involves working with people in a variety of settings such as their own homes, supported living, day opportunities and the wider community.

Main duties of the job

To work with the multi-disciplinary health clinicians under their supervision.

To work with service users to provide interventions to complete specific pieces of work delegated by health practitioners.

To contribute to the regular reviews of service users as part of the multi-disciplinary health team within the LDP.

To participate within the multi-disciplinary professional team to deliver safe person centred care.

To work in partnership with families and carers to ensure that service users individual health needs are met.

To participate in providing health duty with the support of health practitioners

To receive regular supervision from a health practitioner, for line management i.e. Caseload management / annual leave / wellbeing etc.

Detailed job description and main responsibilities

  • To work as part of a Learning Disability multi-disciplinary, integrated team.
  • To support service users with a range of priority health needs under the supervision of a health practitioner.
  • To participate in providing health duty with the support of health clinicians.
  • Participate in regular planning and review meetings with the health practitioner supervising pieces of work and maintain quality and management of caseload within supervision process
  • Maintain adequate records as required by existing procedures, entering appropriate details on the service user’s case notes as necessary.
  • Attend and participate in wider team/care plan review meetings as appropriate.
  • Work with service users to facilitate access to primary and secondary healthcare services.
  • Help to identify early signs of mental and physical health decline by monitoring the service user’s progress, level of functioning and mental state and alert the health practitioner involved in the individuals care.
  • Able to access the local area as this is a community based post which will involve a high amount of travelling.

Education/Qualifications

  • NVQ level 4 OR NVQ level 3 plus training to diploma level or equivalent experience
  • Training in Learning Disabilities

Experience

  • Experience of working with people with a Learning Disability in an Inpatient or community care setting
  • Previous experience of working in a health or social care setting, either statutory or non-statutory service.
