An exciting opportunity has arisen for a highly motivated individual to join the Early Intervention Team (EIT) at Cambridge University Hospitals as a Band 4 Discharge Co-ordinator.
You will be part of an existing team of Discharge Planning Specialist Nurses, Therapists and Frailty Nurses who currently support patients in the Emergency Department and Same Day Emergency Care (SDEC) areas at Cambridge University Hospitals.
Main duties of the job:- The EIT Discharge Coordinator will be based on Emergency Department, Same Day Emergency Care (SDEC) and short stay wards coordinating discharges for all adult patients (18 years and above) to enable a reduction in length of stay, reduction in the number of lost bed days and increase the quality of complex discharge planning whilst supporting members of the MDT team.
- You will track the progress of patients through their hospital journey, and will develop robust plans to facilitate timely discharge.
- You will escalate issues with external organisations, which could cause a discharge delay via Early Intervention Team escalation process.
- Negotiation with staff, services and patients will be a vital part of this role to ensure the patient journey is progressed appropriately and safely.
- You will be responsible for supporting and working alongside EIT Discharge Planning Support Nurses to provide a high standard of assessment, advice and evaluation of patients to formulate appropriate plans of care for patients discharged into Community settings and refer on to appropriate MDT members for patients admitted to the hospital setting.
- The ability to work independently, being able to organise and prioritise your own workload and communicate effectively to a varied audience is essential.

Person specification:- GCSE or equivalent grades A-C in Maths and English (or equivalent).

- Competent with standard IT software programmes e.g. Microsoft Word, Excel etc.
- Ability to use patient information systems e.g EPIC.
