About CHA PACE: Cambridge Health Alliance's Program of All-Inclusive Care for the Elderly (PACE) is a nationally recognized model of care that serves adults aged 55 and older living with chronic medical and/or mental health challenges. At CHA PACE, our mission is to support our program participants in achieving the highest level of independence within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment.
Position Overview: The Registered Nurse is a crucial member of the PACE Interdisciplinary Team (IDT), providing PACE program participants who experience a wide range of chronic healthcare needs with coordinated nursing care in their own homes. The RN demonstrates expertise in the assessment, case management, and implementation of participants’ overall plans of care. They also utilize evidence-based nursing interventions to support these comprehensive care plans, as well as collaborate with the enrollment team in performing initial assessments of participants’ home care needs, such as medication reconciliation and warm hand-offs to other IDT members. In addition, the RN communicates the current status of participants to the IDT, including new problems and/or problems indicating a significant change in status, in order to inform best practices for continued care.
Qualifications:
- Current RN MA licensure required
- AHA BLS Provider certification required; Phlebotomy skills preferred
- B.S. in Nursing preferred
- Reliable transportation to make daily home visits in the CHA PACE service area
- One to three years of home care experience preferred
- Highly developed geriatric assessment and clinical skills
- Experience at a PACE plan, working on an interdisciplinary team, etc. is desired
- Fluency in Spanish, Haitian Creole, Portuguese, etc. is desired
- Clinical experience with geriatric, behavioral health and/or disabled populations is desired
