Cambridge Health Alliance's Program of All-Inclusive Care for the Elderly (PACE) is a nationally recognized model of care that serves adults aged 55 and older living with chronic medical and/or mental health challenges. At CHA PACE, our mission is to support our program participants in achieving the highest level of independence within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment.
Position Overview
We are seeking a highly skilled and dedicated Licensed Practical Nurse (LPN) passionate about making a difference in the lives of senior adults to join our dynamic interdisciplinary team at CHA PACE. In this rewarding role, you will provide direct nursing care to elderly participants both in their homes and in our PACE day clinic, ensuring continuity and quality of care across all settings. We are currently offering a $5,000 sign-on bonus for newly hired PACE LPNs!
Key Responsibilities
- Deliver hands‑on nursing care in participants’ homes and at PACE clinic settings, in collaboration with the RN and PACE interdisciplinary team (IDT)
- Assist in the assessment, planning, and implementation of individualized care plans
- Utilize evidence‑based practices to support participants’ physical and emotional well‑being
- Identify and communicate significant changes in participants’ conditions to the RN and IDT
- Support clinic triage and provide cross‑coverage as needed to ensure uninterrupted care delivery
- Maintain accurate and timely documentation of care and services provided
Qualifications
- Graduation from an accredited licensed practical school of nursing
- 1 year of experience in geriatrics preferred
- 1-2 years of home care experience preferred
- Maintains current Basic Life Support (BLS) certification
- Knowledge of a second language preferred
- Must have a car to travel to participants’ homes
