Cambridgeshire County Council is recruiting three full-time Adult Support Co-Ordinators to join a 12-month programme delivering a new Home Care Framework across Cambridgeshire.
You will conduct strengths-based reviews with adults across Older People, Physical Disabilities, Learning Disabilities and Adult & Autism, collaborating with families, carers, providers and partner agencies to ensure person-centred support and independent outcomes.
