The East Learning Disability Team offers an exciting opportunity for a Social Worker to join us in a permanent role. This part-time position (27.5 hours) is based at Noble House in Ely, with the option for home working as part of a hybrid role. The Cambridgeshire Learning Disability Partnership (LDP) brings together specialist health and social care services for people with a learning disability.
The successful candidates will contribute to this unique model of practice as part of an integrated team alongside Health and Social Care Practitioners such as Adult Support Co-ordinators, Social Workers, Community Nurses, Occupational Therapists, Physiotherapists, Psychologists, Psychiatrist, Speech and Language Therapists and Art / Music Therapists.
What will you be doing?
You will be undertaking Social Care and Mental Capacity Act assessments, completing Safeguarding as well as care and support planning, to achieve better outcomes for the adults we support. You will also be completing Community deprivation of liberty applications to the Court of Protection. You'll need to be inquisitive and provide evidence based, proportionate recording with a keen focus on strength-based practice. As you'll be working with adults, both those in need of care and support services, along with their informal carers, it will be crucial that you work in partnership with them to establish needs and ensure that they are met.
About you
We are looking for people who can demonstrate excellent written and verbal communication skills, have the ability to work both independently and as part of a multi-disciplinary team, and are committed to making a positive difference for the people we support. The successful candidates will have excellent organisational skills and be willing to use their initiative to work in innovative ways.
You must:
- Be an experienced qualified Social Worker with a Social Work England registration
- Can work from a variety of locations and travel countywide independently
