Cambridgeshire County Council is recruiting a qualified Social Worker for a hospital-based role in the South Transfer of Care Team. You will support safe discharges from Addenbrooke's and surrounding hospitals, arranging care and conducting six-week post-discharge reviews to secure lasting outcomes.
You will be part of a multidisciplinary team, travel across the county, and engage in safeguarding when needed. Interviews are likely in late September.
