We are currently recruiting for a Lead Reablement Worker, full time (37 hours) in Fenland. The Reablement Service provides care at home to people aged 18 upwards. Support is delivered in an enabling way so that people can maximise their independence. Reablement is a short term service.
The Lead Reablement Worker plays a crucial role in supporting individuals transitioning towards independent living. They are the first point of contact for service users, working closely with Occupational Therapy teams and other health professionals to set personalised goals. These goals guide the service user through their journey until they achieve independence. Additionally, Lead Reablement Workers act as key contacts, referring users to community support services and continuously monitoring their care throughout the reablement process.
Appointment to these posts will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service. What will you be doing?
The Lead Reablement Worker delivers short-term, person-centered support that helps individuals regain confidence and independence in daily life. This role involves working closely with service users, their families, and other professionals to create tailored reablement plans and risk assessments that promote recovery and reduce reliance on long-term care.
Your day-to-day responsibilities will include:
- Assessing individual needs and creating tailored reablement plans that focus on recovery and independence.
- Working hands-on with service users, helping them relearn essential daily living skills such as personal care, mobility, and meal preparation.
- Monitoring progress and reviewing plans, ensuring goals are met and adapting support as needed.
- Collaborating with health and social care professionals, families, and other agencies to provide a holistic approach to care.
- Maintaining accurate records and documentation to ensure compliance and quality standards.
This role is all about empowering individuals to live independently and with dignity, while effectively managing a varied caseload to ensure timely, high-quality support and positive outcomes.
About you
We treat all of our service users as individuals and put their needs at the heart of what we do. You will have some experience in this kind of work. You will need to have a proficient level of written and spoken English for case recording as well as an NVQ Level 3 or equivalent apprenticeship. Strong communication skills are essential as you will be overseeing Support workers in the community setting and liaising with many health professionals as well as our service users and their families.
As a Lead Reablement Worker you will need to have experience of supporting a person with care and support needs as well as knowledge around The Care Act and CQC requirements. You will be able to demonstrate strong skills in managing and prioritising a diverse caseload, ensuring timely and effective resolution of tasks in a fast-paced environment. You will also have excellent verbal and written communication skills as well as IT skills. It is expected that you will also have the ability to demonstrate awareness and understanding of equality, diversity and inclusion as well as the need for safety and confidentiality in the role.
The role involves travel across Cambridgeshire, and so you will need access to a car, and have a full driving licence with business insurance. You will receive excellent training and supervision as part of an established and supportive team. There is a car lease scheme and we pay enhancements for weekend working. The service covers seven days and so your weekly hours will be on a rota between the hours of 8am and 4.30pm Monday to Friday and will include alternate weekends.
