Cambridge City 4 PCN Ltd. will provide the right candidate with a supportive and nurturing environment with access to continued professional development and career progression opportunities. Working across six Partner practices (registered list size of 59,000) and five aligned care homes, we employ a multidisciplinary team of Clinical Pharmacists and Pharmacy Technicians, Social Prescribers, Care Coordinators, Health Coaches, First Contact Physiotherapists and Paramedics.

Our current Pharmacy team consists of 4 full time Clinical Pharmacists and Independent Prescribers, 2 part time remote locum Clinical Pharmacists, 8 Pharmacy Technicians (3 are currently remote locums) covering 7 whole time equivalents. Each of our 6 Partner practices has a dedicated "team of 2" this being a regular Clinical Pharmacist and supporting Technician. We come together as a team every Thursday at our PCN HUB which provides an opportunity for peer support, CPD, team building/bonding and meetings.

We are keen to enhance the team by recruiting 2 WTE Technicians to replace our locum cover and offer hybrid and flexible working arrangements.

Main duties of the job
  • Working collaboratively with GP practice teams, PCN Clinical Directors, PCN Manager, Lead Clinical Pharmacist, Development and Operational Leads, and community pharmacy colleagues.
  • Medicines reconciliation following hospital discharge.
  • Responding to prescribing and medication queries.
  • Supporting repeat prescribing and medication safety processes.
  • Assisting Pharmacists with medication reviews and structured medication reviews.
  • Supporting audits and medicines optimisation initiatives.

Job responsibilities
  • Hybrid model including 2 practice-based days, 1 hub day and 1/2 remote working days.

Person Specification
  • Registered Pharmacy Technician with the General Pharmaceutical Council.
  • Qualified and experienced vaccinator.
  • Experience working in pharmacy practice.
  • Strong communication and organisational skills.
  • Ability to work independently and within a multidisciplinary team.
  • Clean and current drivers licence.
  • Experience of working in Primary Care.
  • Experience and knowledge of Systm1 clinical system.
