Advanced Care Practitioner/ Advanced Nurse Practitioner in Oswestry

We are seeking a dynamic and highly motivated ACP or ANP to join our team, working either full or part time. The postholder will undertake a varied range of clinical duties, supported by our experienced clinical and administrative teams. Working autonomously within your scope of practice, you will assess, diagnose, treat, and manage patients with a range of acute and long-term conditions, playing a pivotal role in delivering high-quality primary care.

The ACP or ANP will play a key role in delivering high-quality clinical care within the practice. The role involves the assessment, diagnosis, treatment, and or referral of patients presenting with undifferentiated and undiagnosed conditions. The postholder will work independently within their professional scope of practice, with regular debriefs and access to a supervising GP for advice throughout the day.

• Deliver a high standard of patient care using advanced autonomous clinical skills supported by a broad and in-depth theoretical knowledge base.
• Assess, diagnose, and manage patient conditions, initiating appropriate referrals when required.
• Manage acute illnesses, long-term conditions, and minor ailments confidently and safely.
• As an independent prescriber, prescribe safe, effective, and appropriate medication in line with legislation and professional guidelines.

We are a well-established, friendly, and supportive GP surgery serving approximately 13,500 patients across a large rural catchment area and are proud to be part of the North Shropshire Primary Care Network. Our multidisciplinary team includes GPs, Advanced Nurse Practitioners, Practice Nurses, a Nurse Associate, Healthcare Assistants, Clinical Pharmacists, and allied health professionals such as a Cancer Care Coordinator, First Contact Physiotherapist, and Paramedic. Together, we deliver high-quality, patient-centred care in line with the NHS model of modern general practice.

The ACP or ANP will work autonomously within their professional boundaries, providing high-quality clinical assessment, diagnosis, treatment, and evaluation to patients presenting with both acute and long-term health conditions. The post-holder will manage undifferentiated and undiagnosed presentations, making complex decisions based on advanced clinical knowledge and skills. Working as part of a supportive multidisciplinary team, the ACP or ANP will contribute to safe, effective, and patient-centred care, supporting the practice's commitment to high clinical standards and continuous service improvement. Regular debriefs and access to a supervising GP will be available throughout the working day.

• Previous primary care experience, specifically working within a GP practice nursing team.
• Experience of NHS IT System EMIS.

• Experience of clinical audit evaluation research and implementation of evidence-based practice.
• Understanding of national and local guidelines.

• Hold one or more academic Level 6 diploma or postgraduate certification in long-term conditions care and or public health initiatives.
• Registered with the NMC and maintains revalidation in line with NMC requirements.
• Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
• Enhanced DBS check.
• Evidence of post-qualifying and continuing professional development.
• Valid UK driving licence.
