The junior clinical pharmacist will work across the CAM Medical Primary Care Network area in the pharmacy team.

Job responsibilities include:
- Responding to pharmacy queries, including clarifying doses and/or products and providing appropriate alternatives when availability issues arise.
- Discussing medication issues with patients as needed, including adverse effects, interactions, and queries related to over-the-counter remedies.
- Conducting medication reviews and reviewing repeat medications for new patients registering with the practice, suggesting GP medication reviews where appropriate.
- Face-to-face/video consultations with patients will include discussing medication issues as needed and conducting medication reviews.
- Reviewing secondary care requests for new medications, raising queries with relevant GPs and addressing any inappropriate prescribing requests.
- Discussing medication changes post discharge with patients, if necessary.
- Collaborating with the practice team to identify patients at risk of unplanned admissions.
- Monitoring and informing colleagues about ongoing prescribing issues, including new guidelines, products requested by secondary care, and any manufacturing or supply problems.
- Advising on cost-effective prescribing and budget issues.
- Assisting in identifying patients in need of medication review and developing systems to meet these needs.
- Working with GPs and practice teams to implement safe and efficient prescribing policies.
- Identifying national and local policies and guidance that impact patient safety regarding medicines.
- Supporting the integration of CAM Medical Primary Care Network with community and hospital pharmacies.
- Coordinating pharmacy services across three practices, ensuring effective communication among teams.
- Maintaining accurate records of medication reviews, consultations, and patient interactions.
- Attending training sessions and meetings for pharmacy and PCN staff across all practices.
- Supporting the equality, diversity, and rights of patients, carers, and colleagues.
- Participating in training programs implemented by the Practice, including undertaking the Clinical Pharmacist CPPE course.
- Engaging in annual performance reviews, maintaining a record of professional growth.
- Striving to maintain quality within the Practice by alerting team members to quality and risk issues.
- Recognising the importance of effective communication and striving to communicate clearly with team members, patients, and carers.
- Applying practice policies, standards, and guidelines, discussing their impact on work with other team members.
- Accessing confidential information regarding patients, their carers, and CAM Medical Primary Care Network staff according to practice policies and procedures.
