Supports clients through their recovery journey, both clinically and holistically, while maintaining high standards of care and contributing to service development and innovation.

Responsibilities:

  • Dispense controlled and non‑controlled drugs to meet clients’ physical support needs.
  • Conduct clinical and psychosocial assessments to create person‑centered integrated care plans.
  • Deliver health promotion work in areas such as sleep management, BBV, wellbeing, detox promotion and plan setting, new initiatives and monthly topics.
  • Achieve KPIs in line with contract agreements to evidence service delivery.
  • Deliver awareness sessions linked to substance misuse.
  • Collaborate jointly with multidisciplinary stakeholders such as healthcare, offender management, safer custody, community CDTs and CPNs.
  • Manage a caseload of complex and non‑complex clients.
  • Support colleagues and managers with new initiatives and service offers, and any reasonably requested activity.
  • Plan care for clients’ recovery journeys, keeping the client at the centre of all decision making.
  • Supervise support workers.
