Position Summary

The Seniors’ Wellness Facilitator is responsible for the planning, coordination, and delivery of group-based wellness programs for older adults.

This includes social, recreational, and educational activities that support physical health, mental well-being, and social connection.

Working as part of the Caledon Meals on Wheels team, the Facilitator ensures programs are welcoming, safe, and responsive to participant needs.

The role supports client engagement, volunteer involvement, and consistent program delivery in alignment with organizational policies, procedures, and accreditation standards.

Please note this role is for a 28-30 hours weekly, Monday, Tuesday, Thursday and Friday, 8:30am – 4pm.

Key Responsibilities and Duties

  • Program Planning and Delivery
  • Plan, schedule, and deliver a range of wellness programs, including physical activity, social, and educational sessions.
  • Adapt programs to meet varying abilities, interests, and needs of participants.
  • Prepare program materials, set up and take down equipment, and ensure spaces are safe and welcoming.
  • Monitor participation and engagement, and adjust programming as needed to improve experience and outcomes.
  • Client Engagement and Support
  • Build positive, respectful relationships with participants and create an inclusive group environment.
  • Welcome new participants and support registration and orientation to programs.
  • Observe and respond to participant needs, including identifying changes in well-being and escalating concerns as appropriate.
  • Support a dignified, respectful, and client-centred approach in all interactions.
  • Volunteer Support
  • Work alongside volunteers during program delivery, providing guidance and support as needed.
  • Help ensure volunteers understand their roles, responsibilities, and boundaries.
  • Promote a positive and respectful environment for volunteer involvement.
  • Program Administration and Documentation
  • Maintain accurate records of attendance, program activities, and participant feedback.
  • Support basic data collection to inform program improvements and reporting.
  • Assist with scheduling, communications, and coordination of program logistics.

  • Follow organizational policies and procedures related to health and safety, confidentiality, and client care.
  • Identify and report incidents, concerns, or risks in a timely manner, following established processes.
  • Support safe program environments, including equipment use and participant mobility considerations.
  • Community Engagement
  • Support outreach efforts by promoting programs through community events, partnerships, and internal channels.
  • Maintain awareness of community resources and refer participants when appropriate.

Qualifications

Education and Experience

  • Diploma or degree from an accredited college or university in Recreation, Gerontology, Social Services, Health Promotion, or a related field, or equivalent experience.
  • Minimum 1–2 years of experience working with older adults in a community or program setting.
  • Experience facilitating group programs or activities is strongly preferred.

Skills and Knowledge

  • Strong interpersonal and communication skills.
  • Ability to lead group activities in a confident and engaging manner.
  • Understanding of aging, wellness, and social determinants of health.
    Basic proficiency with Microsoft 365 or similar tools.
  • Other Requirements
  • Valid driver’s licence and access to reliable transportation.
  • Satisfactory Vulnerable Sector Check.
  • Working Conditions
  • Regular movement between program locations.
  • Moderate physical activity, including lifting and setting up equipment.
  • Frequent interaction with older adults, some with complex health or social needs.
  • Occasional evening or weekend programming may be required.
