Located near Savile Park in Halifax, Heatherstones Court is a welcoming reablement and specialist housing service supporting people from 18 years of age and above to regain confidence and independence. Our modern setting includes eight single apartments and three double apartments, providing a safe and comfortable environment where people can receive high quality primary care during their stay.

We’re looking for a dedicated and compassionate Reablement Support Worker with proven experience in medication good practice to join our friendly, supportive team.
Heatherstones Court provides support for:
- People on discharge from hospital who need a short period of help and support (Reablement) before getting back to their former self.
- People who are experiencing difficulties in their own home and can no longer access their home because of changes to their health or personal circumstances.
- People who have become homeless because they are unable to return to their own home.
- People may be living in a property which requires urgent repairs or has been assessed as unsafe to reside.

Through a holistic approach to care, we aim to address not only the physical aspects of reablement but also the emotional and psychological well‑being of the people we care for.
As a Reablement Support Worker, you’ll work closely with individuals recovering from illness, injury, or hospital stays, supporting them to rebuild skills, confidence, and independence in their own homes. You’ll:
- Encourage and guide people to carry out daily living activities.
- Support safe and accurate medication administration in line with best practice.
- Promote wellbeing, dignity, and choice always.
- Work collaboratively with healthcare professionals, families, and carers.

Job responsibilities
Join our multidisciplinary, multi‑agency team supporting adults of all ages to stay independent in their own homes and communities. This role will deliver medical, nursing, therapy, or social care support that promotes dignity, choice, and independence without the need for hospital admission.
The post holder will work directly with people in their own homes across the Calderdale area to help them live well and remain safe in their community.
The post holder will be expected to work flexible working patterns as part of a continuous rota between the hours of 8 am and 10 pm everyday throughout the year, 7 days a week, predominantly early morning and evenings, but this is not exclusive.
Establish positive relationships with the people we provide care for within professional boundaries, promoting dignity, choice, and respect.
Be alert for the changing needs and conditions of the people we visit, reporting concerns to management, particularly where the person appears to be at an increased risk of harm occurring.
Recording/Documentation accurately. To read, respond and act upon care plans, risk assessments, manual handling assessments, medication profiles and any other written information within the person’s home. IT skills by completing mileage, booking A/L, toil and applying for training on HR system.
Deliver care and support as identified in Care Plans, support plans and risk assessments. This may include intimate personal care as well as supporting the person with daily routines and tasks.
Assist people, where appropriate, to manage and take their prescribed medication in accordance with their individualised medication profile and medication policy, to be competent in dispensing medication from boxes and bottles, apply pain patches, contact chemist re medication, collection, and delivery prescriptions.
Community participation: To work with the individual and their families to identify community activities that will support the individual to maintain their connections with themselves, the people and the environment that surrounds them.
Attend meetings, reviews, and case conferences. Participating in the development of support plans, risk assessments and live story work that captures and remembers the individual.
Attend training as identified through appraisal, in line with national minimum care standards, Skills Training and Council policy, and be pro‑active in any development opportunities as they arise. To assist in the induction of new colleagues and to maintain positive relationships within the team and beyond.
Work as part of a multi‑disciplinary team, in co‑operation with other carers and professionals, whilst maintaining confidentiality with regard to service user condition, personal and monetary affairs.
Support and participate in treatment/exercise regimes and rehabilitation plans identified by other team members undertaking basic nursing, occupational therapy and physiotherapy roles following appropriate training, i.e. putting a splint on foot and following the exercise regime laid out, use of walking aids, deliver and fit equipment.
Be competent in assisting a service user with stoma care, and catheter care, to recognise risks and report concerns to the appropriate person.
Initiative, to have a responsible attitude and a creative approach, to be open to try new ways of working with service users and their carers. The ability to work alone and use your own initiative, however, also raise any issues that you feel need supervisory support and may have to deal with any emergency.
Person Specification
Qualifications
- Level 2 qualification in Health and Social Care (or equivalent) or willing to work towards.
Experience
- Basic understanding of the needs of people who need care at home.
- Basic understanding of the needs of people with life changing disabilities. E.g. stroke, amputation, reduced mobility, wheelchair users, including an understanding of promoting independence and choice to the individual.
- Be aware of and promote good practice with regard to:

- Experience of lone working.
- IT skills relating to Microsoft products and in‑house systems.
- Basic understanding of Person‑Centred Approaches.
- Communication Skills Verbal, non‑verbal and written.
- Supporting individuals who require care and support.
- Following instructions, adapting to short notice changes.
- Understanding of the monitoring processes in relation to maintaining positive CQC outcomes.
- Knowledge of Substance abuse and self‑neglect.
- Knowledge and experience of supporting people with their medication in a safe and timely manner, and how to record this effectively.
- Practical experience of using equipment relating to Care.
