Overview

The Early Help Hub forms part of Calderdale’s front door for Early Help Support. It works collaboratively across the borough with a range of services to provide support to children, young people and their families. The Hub responds to referrals made through the Multi-Agency Screening Team (MAST) that don’t meet the threshold for Children Social Care involvement. It is a multi-agency decision making forum that assesses levels of need to ensure the right services and interventions are provided to meet identified needs at the right time.

The post holder will offer immediate short-term support for up to 12 weeks, working intensively with targeted families who face particularly complex and multiple challenges. Using a holistic \”think family\” approach, they will carry out assessments of needs, provide targeted interventions and bring together a coordinated package of support. They will manage a complex caseload, including those subject to Early Help assessments, Child In Need, Child Protection and Children with Disabilities, in accordance with procedures and the support of Children’s Social Care.

Responsibilities

  • Provide immediate short-term support for up to 12 weeks, working intensively with targeted families with complex and multiple challenges.
  • Conduct holistic needs assessments using a \”think family\” approach and coordinate a package of targeted interventions to meet identified needs.
  • Manage a complex caseload, including cases under Early Help assessments, Child In Need, Child Protection and Children with Disabilities, in collaboration with Children’s Social Care.
  • Participate in the multi-agency decision making process to determine appropriate services and interventions.
  • Act as Lead Professional where required, offering both support and challenge within the required framework.

Qualifications and qualities

  • Experience of working with hard-to-reach children and families through an outcomes-focused approach.
  • Strong assessment, analytical and reflective skills.
  • Ability to work as a Lead Professional and operate within a framework of support and challenge.
