A Vacancy at Calderdale and Huddersfield NHS Foundation Trust. This is an exciting opportunity for someone to work in the field of respiratory rehabilitation including patients with long covid syndrome. To provide delegated rehabilitation plans to people with chronic respiratory disease in a variety of environments including groups and home base rehabilitation. To provide administration support to both pulmonary rehabilitation and the post covid service.
Activities will include:
- Implementing exercise programmes to individuals or groups.
- Assisting with exercise assessments of pulmonary and long covid patients.
- Assisting with and or completing the exercise assessments at point of discharge.
- Making minor adjustments to treatment programmes within pre-determined limits.
- Assisting registered practitioners with complex treatments.
- Assisting with group exercise and advice sessions.
- Utilising the SystmOne clinical information system to retrieve patient information, register referrals and discharges, deal with tasks, type and send letters.
- Preparing responses/letters/acknowledgments and requests for the Pulmonary Rehabilitation clinical lead and therapy staff.
- Ensuring written documentation is accurate and disseminated to other professionals within the Trust, external agencies and carers.
- Providing a first point of contact for service users, answering the telephone and dealing with enquiries in a sensitive manner to document and convey messages to the team.
- Arranging appointments for clinic consultations, home visits and meetings face to face and/or virtual.
- Checking email accounts, processing referrals and amending waiting lists for both pulmonary rehabilitation and the post covid service.
- Attending post covid multidisciplinary team meetings and completing patient templates and any administrative duties post meeting.
- Printing patient assessment packs including exercise sheets and information booklets.
The post holder will support the delivery of Pulmonary Rehabilitation in Calderdale. They will undertake highly skilled support work with people who have respiratory disease and complex multi-pathology conditions including long covid. The work will involve assisting with the Pulmonary Rehabilitation groups face to face and virtual, contacting patients, and assisting with the office administration for the Pulmonary Rehabilitation Service and post covid service. You will also deliver one to one rehabilitation programmes to patients within their own homes. The post holder will (in allocated time) act as a point of contact for all communication for the team, answering phones and organising diaries.
