The Laurandy Centre is recruiting an experienced Care Planning Coordinator to join our team. The Laurandy Centre is a registered day support service for older people, delivered by a respected third sector organisation and funded as a commissioned service by the NHS. We have an excellent reputation as a trusted local care provider.

Working closely with the Manager, you will support the delivery of high-quality, person-centred care, maintain and develop care planning systems, and ensure the safety, wellbeing, and best outcomes for service users. You will also support quality monitoring through regular audits and help promote the service locally.

Requirements:

  • SVQ Level 4 (SCQF Level 9) in Social Services and Healthcare or equivalent
  • Experience in a senior care role
  • Ability to work independently and use initiative
