We are seeking a Qualified Social Worker to join the Discharge to Assess (D2A) team in multiple locations, including Bristol, Bournemouth, Manchester, and Staffordshire. The role involves supporting adults transitioning from hospital to home, ensuring safe, timely discharges, developing tailored care plans, and providing ongoing support.

Responsibilities

  • Conduct comprehensive assessments of individuals' social care needs prior to discharge.
  • Develop and implement tailored care plans in collaboration with healthcare professionals, families, and community organisations.
  • Provide ongoing support and monitoring to ensure successful implementation of care plans.
  • Advocate for clients' rights, promote independence and well-being.
  • Maintain accurate records and documentation in accordance with regulatory requirements.
  • Collaborate with multidisciplinary teams to promote seamless transitions and facilitate discharge planning.

Qualifications

  • Recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registered with Social Work England.
  • Proven experience in adult social work, particularly in discharge planning or community care.
  • Strong communication and interpersonal skills, able to build rapport with clients and stakeholders.
  • Excellent organisational skills and ability to manage a varied caseload effectively.
