Caerphilly County Borough Council is seeking a Social Worker to join the Hospital Discharge Team. The role focuses on supporting Caerphilly residents to achieve safe and timely discharge from hospital within the Hospital Discharge Service, part of the Community Resource Team.

You will need a Diploma/ Degree in Social Work and be registered with Social Care Wales (or eligible to transfer). A full UK Cat B driving licence is required to travel to sites across the borough.
