We are looking to recruit an Advanced Nurse Practitioner to join our GP Practice. This is an excellent opportunity for an ANP who is passionate about delivering high patient centred care and working collaboratively within a multi disciplinary primary care team. You will work alongside GPs, nurses and the wider practice team.
About you:
- Registered Nurse with a recognised Advanced Nurse Practitioner qualification.
- NMC Registered.
- Prescribing Qualification.
Main duties of the job:
- Undertake advanced clinical assessments, diagnosis and management of patients.
- Prescribe medications independently and review treatment plans.
- Make appropriate referrals to secondary care and community services.
- Maintain accurate and timely clinical records.
- Ensure care is delivered in line with national and local guidelines, safeguarding policies and professional standards.
- Work autonomously while contributing to team decision making.
About us:
We are an established GP surgery providing patient care to the local community. We have around 4800 patients registered at the surgery which is based in Dolgellau rural Wales. We currently have three GPs, 2 ANPs, 2 Practice Nurses and 3 health care assistants, as well as a supportive administration and reception team. We are a very supportive team and offer a friendly working environment.
Job responsibilities:
- Deliver quality care within the GP Practice.
- Provide advanced clinical assessment, diagnosis and management of patients.
- Manage same day and routine appointments in line with practice policies.
- Prescribe medications independently and review treatment plans.
- Request, interpret and act on diagnostic investigations.
- Refer patients appropriately to secondary care and community services.
- Work collaboratively with GPs, ANPs, Practice nurses and allied health professionals.
- Ensure care is delivered in line with national guidelines, safeguarding requirements and professional standards.
Person Specification:
- MSc Advanced Practice Qualification.
- NMC Registered.
- Experience of working in a GP surgery or community setting.
- Evidence of continued professional development relevant to primary care.
