Job Purpose
Provide clinical leadership to improve cancer pathways for global majority communities by bridging hospitals, primary care and community services, advocating for patients and influencing system-wide improvements.
Key Responsibilities
- Provide personalised advocacy and support throughout the cancer pathway.
- Support shared decision-making and explain diagnosis and treatment options.
- Coordinate referrals across hospital, primary care and community services.
- Identify and escalte pathway failures and barriers to care.
- Work with NHS, VCSE and community partners to improve continuity of care.
- Contribute to pathway mapping, service redesign and health equity initiatives.
- Collect monitoring data and contribute to evaluation and reporting.
Person Specification Essential
- Registered healthcare professional (NMC, HCPC or equivalent).
- Experience within cancer services or long-term condition management.
- Knowledge of NHS pathways and health inequalities.
- Excellent communication, advocacy and partnership-working skills.
Desirable
- Experience working with global majority communities.
- Experience in quality improvement or service redesign.
