Northfield Community Partnership is seeking a Lifestyle Coach. The closing date is upon the appointment of a candidate.
The Lifestyle Coach will support individuals with type 2 diabetes to transform their health through personalised, locally delivered and culturally adapted support. The role will work directly with participants to enable them to achieve their goals.
Salary: £30,000 - £33,000 Annually
Hours: 34 hours per week
Contract: Until March 2027
Holiday: 33 days holiday annually (including bank holidays), pro rata
Location: Office-based and across the Northfield district
Main Duties and Responsibilities:
- Manage triage of referrals, ensuring that only participants suited for the service are enrolled, and refer others to more suitable programmes as needed.
- Complete baseline assessments with eligible participants to ascertain their starting point.
- Work with eligible participants to create personalised action plans aimed at supporting them to achieve goals; covering weight reduction, physical activity and nutrition.
- Help participants navigate various health services and local activities based on their needs.
- Where appropriate, introduce service users to community groups, activities and statutory services, ensuring they are comfortable.
- Follow up to ensure they are happy, able to engage, included and receiving good support.
- Meet participants on a one-to-one basis at agreed intervals.
- Actively manage case load, maintaining regular contact to ensure participant goals are met and overall service outcomes are achieved.
- Deliver health-related workshops/group sessions as needed for the programme.
- Outreach for participants, actively engaging and connecting with individuals in the community, using targeted communication and partnerships to raise awareness and encourage their involvement.
- Understand and use CRM for all service users and update as required.
- Ensure all data, monitoring and evaluation is kept up to date and reported back as required.
- Adhere to data protection, safeguarding, confidentiality, information sharing protocols as required.
- Participate in professional supervision, as required.
Person Specification:
Qualifications:
- RSPH Level 3 Certificate in Health Coaching or equivalent experience and/or qualification (E)
- Community, health, social care, counselling or other relevant experience and/or qualification (D)
- English and Maths GCSE or equivalent (D)
Knowledge and Understanding:
- A good understanding of personalised care (E)
- Understanding of evidence-based behaviour change principles and practice how a holistic approach to supporting service users leads to behaviour change (E)
- Strong understanding of barriers to accessing health and local community services and how to navigate them (E)
- A good understanding of ethnically diverse communities (E)
- A proven understanding of safeguarding for children and vulnerable adults (E)
Experience and Skills:
- Previous experience in health or community-related role supporting individuals to change their health or lifestyle (E)
- Experience working collaboratively within multidisciplinary teams and a confident approach to working with a variety of stakeholders (E)
- Excellent written, verbal, listening and presentation skills (E)
- Excellent IT skills and ability to do own administration using database and other IT packages (E)
- Proven skills in collating information and data on community resources and organising it in up-to-date and accessible formats (E)
