As an Advanced Practitioner, you will provide practice leadership within the Adults with Learning Disabilities team, supporting the delivery of high-quality, strengths-based interventions. You will support the Team Manager and Assistant Team Manager in driving excellent standards of practice, while holding a reduced but complex caseload, including individuals with multiple and high-level needs. You will also supervise a small number of staff and have strong strength-based practice with the ability to provide professional challenge and support.
Key Responsibilities
- Provide expert practice leadership, mentoring, and reflective support to social workers and practitioners
- Lead on complex casework, including safeguarding, risk management, and multi-agency interventions
- Champion strengths-based, person-centred practice in line with the Care Act 2014
- Apply and support others in using the Mental Capacity Act (MCA) 2005 and Best Interest decision-making
- Complete S16 Welfare Applications and lead on S21A Applications
- Support quality assurance, audits, and continuous service improvement
- Work closely with health partners (including CLDTs who we are co-located with), providers, and community services
- Promote independence, choice, and control, supporting people to achieve meaningful outcomes
- Contribute to service development and innovation within learning disability provision
