The post holder will be accountable for ensuring the higheststandard of clinical care is provided within the relevant setting. To work asan autonomous nurse prescriber, responsible for the delivery of nursingservices, working as part of the practices multidisciplinary team, deliveringcare within their scope of practice, to the entitled patient population.
The post holder emphasis will be on patient safety, patientexperience, patient engagement, and clinical outcomes. The postholder will have clinical responsibilityfor supporting the partners in activities relating to the delivery of high-qualityservices, including involvement in maintaining activity levels, managing servicepressures and maintaining the highest standards of care.
The post holder will be responsible for a number of clinical areas such as minor illness clinics,health promotion, Diabetes care, chronic disease management, health prevention,as well as supporting the partners and clinical teams in reviewing clinicalpolicy, protocols and procedures to deliver high quality sustainable care forpatients.
The post holder will have developed skills across a widerrange of subject matter, including diagnostic and therapeutic interventions.
Main duties of the job

Job responsibilities

Manage patients presenting with a range of acuteand chronic medical conditions, providing subject matter expert advice topatients.
Evaluate and implement individual specialisttreatment plans for chronic disease patients.
Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects to the patients\βhealth.
Provide wound care.
Provide specialist clinics delivering patientcare as necessary, referring patients to secondary/specialist care as required.
To complete medication authorisations forpatients.

Take calls from outside members of themulti-disciplinary teams, work with external agencies.
Participate in the review of significant andnear-miss events applying a structured approach i.e. root cause analysis.
Clinical Quality and Patient Experience
Address the concerns of patients and servicesusers and respond to their suggestions for local quality improvements inservices.
Work with the Practice Manager and clinicalteams to ensure the effective resolution of complaints and any subsequentlearning, undertaking risk management and reviewing patient experience as necessary.
Act as a positive clinical role model. Lead by example to motivate others, ensuringthe highest standard of care.
To work with the clinical team to ensure thestandards of cleanliness and hygiene in their areas comply with policies andprocedures.
Support the development of protocols, processand best practice to ensure the highest standard of care.
Maintain accurate clinical records inconjunction with extant legislation.
Support on CQC compliance for clinical bestpractice, ensuring process and policies align to CQC compliance standards.
Support the Practice Manager with developing andmaintaining robust CQC standards, reviewing current processes and policies.
Work with the Practice Manager on Quality andcompliance standards, issues and risk.

To support the delivery of Safeguarding Adultsand Children Governance framework and processes within the organisation. Encourage the clinical team to promptly elevate highly complex safeguarding cases to the Practice Manager.
