About the role
The Targeted Support Coordinator is responsible for providing practical and emotional support to vulnerable customers with a history of rough sleeping. You will build strong, professional relationships to maximise engagement, reduce barriers, and minimise tenancy risks. You will manage a small, targeted caseload (anticipated maximum of 10 customers), which may vary depending on the complexity of individual needs.
A typical day may include:
- Conducting needs assessments with vulnerable and at-risk customers to inform effective, person-centred support plans.
- Completing risk assessments, support planning, goal setting, and regular reviews.
- Providing and coordinating tailored support, advice, and guidance to help customers sustain their tenancies and access health, care, and support services.
- Supporting customers to build social networks, confidence, independence, and self-esteem.
- Sourcing furniture and essential household items through grants and charitable funding.
- Liaising closely with colleagues, partner agencies, external organisations, and family or support networks to coordinate specialist support and referrals.
- Assisting customers with daily living tasks such as cooking, shopping, budgeting, tenancy sustainment, and moving on to independent accommodation where appropriate.
- Spending quality time with customers through regular catch-ups and informal support—this role prioritises meaningful engagement rather than rushed appointments.
- Producing quarterly case studies and outcome reports to evidence customer progress.
You will bring:

- Previous experience working with vulnerable and complex individuals in a similar or related setting (lived experience will also be considered).
- An understanding of social housing, tenancy sustainment, and welfare benefits.
- Strong partnership-working skills, with proven experience of multi-agency collaboration.
- Effective communication, engagement, and negotiation skills.
- A full driving licence and access to a suitable vehicle insured for business use (mileage expenses are reimbursed).
