About the Role As a Outreach Worker , you will be at the forefront of supporting individuals experiencing homelessness, rough sleeping, or social exclusion.
You’ll engage directly with people in the community, often in non-traditional settings, to offer support, build trust, and connect them with vital services.
Your role will focus on: Actively engaging with people who are vulnerable, isolated, or hard to reach.
Providing tailored advice and support around housing, health, benefits, and wellbeing.
Encouraging individuals to access appropriate services and take steps toward independence.
Working in partnership with local agencies, charities, health providers, and housing services to ensure wraparound support.
Maintaining accurate records and contributing to a team approach in delivering high-quality support.
