Embark on an exciting journey with a leading mental health charity as a Support Navigator! In this dynamic contract role, you will be the guiding light for individuals navigating the complexities of community-based services. You will be working directly with complex mental health issues, with the opportunity to pick up a sleep in shift a week with a competitive hourly rate of £12.21 per sleep (across 12 hours - £146.52 per shift).
Standout Highlights:- Manage a caseload of service users with learning disabilities, complex mental health needs, and neurodivergence.
- Collaboratively create, update, and review risk assessments and support plans.
- Support with crisis intervention & management.
- Work in the community across x2 services and sometimes within people’s homes.
- Exceptional communication skills to effectively liaise with diverse stakeholders.
- Strong problem-solving abilities to navigate complex systems and find solutions.
- Empathy and compassion to provide a caring and supportive experience.
- Familiarity with community-based services and resources.
- Proficiency in data management and record-keeping.
