Occupational Therapist - Healthy Homes Project

Location: Bridgend

Hours: 15 hours per week

Salary: £39,592 pro rata (£20.58 per hour + £2.58 per hour holiday pay)

Contract: Fixed term, till March 2027 (good potential for extension)

An exciting opportunity has arisen for a part-time Occupational Therapist to join a Healthy Homes Project, providing a preventative service that supports older and disabled people to remain safe, independent and well in their own homes.

The successful candidate will undertake holistic Occupational Therapy assessments within clients' homes, responding to direct referrals from GP practices and working closely with colleagues across health, housing and social care.

Key responsibilities

  • Undertake holistic assessments of clients' physical, cognitive and emotional needs, using recognised assessment tools.
  • Develop and deliver personalised plans that promote independence and participation in daily activities.
  • Recommend and source appropriate aids, equipment and environmental adaptations to support individual goals.
  • Work collaboratively with clients, families, carers and multidisciplinary teams to provide a holistic service.
  • Maintain accurate and timely records.
  • Work closely with Caseworkers and partners across the health, housing and social care sectors.

About you

  • You will be a qualified Occupational Therapist with current HCPC registration, strong clinical reasoning skills and a genuinely client-centred approach.
  • You will be confident working independently in clients' homes and able to work collaboratively with a range of professionals, families and carers.

Essential requirements

  • Relevant Occupational Therapy qualification.
  • Current HCPC registration.
  • Full UK driving licence and access to a car.
  • Ability to work in a client-centred and preventative way.
  • Successful completion of a DBS check.
