We have exciting opportunities for you to join our All Age Complex & Continuing Care (AACCC) team, working across Buckinghamshire and Berkshire West. We are looking for highly motivated Registered Nurses of all disciplines AND/OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment to support the assessment and review process for NHS Continuing Health Care (CHC) and NHS Funded Nursing Care (FNC).
Prior knowledge of AACCC would be beneficial but is not essential as you will work within an experienced and supportive team who will support you as you settle into your new role. Support will be available from colleagues and senior management to help you develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively.
At the same time, as you demonstrate your expertise and skills you will have autonomy to plan and prioritise your own workload to meet the requirements of the post. There is the opportunity to work full time - or part time - on a hybrid basis and in addition we support flexible working options to enhance the work/life balance of our teams. We operate within core business hours, Monday to Friday.
