We have excitingopportunities for you to join our All Age Complex & Continuing Care (AACCC)team, working across Buckinghamshire and Berkshire West.

We are looking for highlymotivated Registered Nurses of all disciplines AND/OR Allied HealthProfessionals who can work autonomously and collaboratively in a multi-agencyenvironment to support the assessment and review process for NHS ContinuingHealth Care (CHC) and NHS Funded Nursing Care (FNC). Prior knowledge of AACCCwould be beneficial but is not essential as you will work within an experiencedand supportive team who will support you as you settle into your new role.

Support will be availablefrom colleagues and senior management to help you develop the necessary skillsand experience to work across the whole pathway, safely, confidently andeffectively. At the same time, as you demonstrate your expertise and skills youwill have autonomy to plan and prioritise your own workload to meet therequirements of the post.

There is the opportunityto work full time – or part time – on a hybrid basis and in addition we supportflexible working options to enhance the work/life balance of our teams. Weoperate within core business hours, Monday to Friday, excluding bank holidays.

Main duties of the job

You will be involved in patient assessment and reviews, care planning and case management as well as audit, appeals, retrospective assessments, complaints, assessment verification, and much more.

The post holder will beresponsible for undertaking specialist health needs assessments for individualswho are undergoing assessment for, or who are already eligible for, NHSContinuing Healthcare and NHS Funded Nursing Care.

Assessments are generallycarried out within the community in residential settings but may also take ahybrid approach. Therefore, you will need to have access to a car to supportyou with your role. Travel expenses will be reimbursed in line with NHS Policy.

You will be required towork with the individual and/or their representative, and to liaise withprivate providers, partner organisations, along with other health and socialcare professionals to ensure that assessments are person centred.

Job responsibilities

Person Specification

Communication Skills

  • Must be able to provide and receive sensitive or contentious information, negotiate with individuals, their families and representative and stakeholders on challenging and controversial issues.

Experience

  • An awareness of CHC National Framework.
  • An understanding of current healthcare policy and an appreciation of how it impacts on patient engagement and services.
  • Should have an appreciation of the relationship between the Department of Health & Social Care, local authorities and individual provider and commissioning organisations.
  • Working knowledge and experience of CHC through prior work experience.

Analytical

  • Problem solving skills and ability to respond to sudden unexpected demands.
  • Ability to analyse complex facts and situations and develop a range of options.
  • Take decisions on difficult and challenging issues where there may be a number of courses of action.
  • Strategic thinking ability to anticipate and resolve problems before they arise.

Planning Skills

  • Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly.

Autonomy Freedom to Act

  • Must be able to use initiative to decide relevant actions and make recommendations to Team Manager, with the aim of improving deliverables and compliance with policies.
  • Ability to make decisions autonomously, when required, on difficult issues, working to tight and often changing timescales.
  • Experience of identifying and interpreting national policy.
  • Experience of researching best practice (globally, private and public sector), interpreting its relevance and processes/ practices which could be implemented successfully to achieve service improvement.

Qualifications

  • Hold a graduate level or equivalent qualification in a relevant health or care discipline.
  • Evidence of post qualifying continuing professional development.
  • Member of relevant professional body.

Management Skills

  • Must be able to prioritise own work effectively. Experience of managing and motivating colleagues.
