District Nurse/Community Case Manager Nurse – Buckingham ACHT
The Buckingham Adult Community Health Care Team (ACHT) is recruiting for a Band 6 District Nurse position. The ACHT uses an integrated approach designed to meet the changing needs of the local population, with emphasis on proactive community care, including caseload management, prevention of admission, reablement and end of life care. The successful candidate will facilitate timely discharge from hospital using a coordinated approach to care in partnership with key care partners. The ACHT provides a seven days a week service and a shift pattern will be required.
Responsibilities
- Co-ordination and ability to lead a team within a primary healthcare setting utilising an integrated approach to care.
- Setting, maintaining and delivering high standards of care in a variety of locations.
- Ensuring rapid access to complex care provision to improve end of life care.
- Case load management: assessment, implementation and evaluation of individualised evidence based care plans.
- Working in partnership with other agencies to address the needs of the patient.
- Leadership/management role within a designated team.
Qualifications/Experience
- Qualified District Nurse or experienced Community Nurse with community-based experience (development role).
