An exciting opportunity has arisen for an aspiring individual to develop their career in the specialised area of District nursing. This opportunity is now available in the Chalfont's and Gerrards Cross area to become a Community Nurse within a newly formed team. We are welcoming highly motivated and effective team players, committed to providing a high quality nursing service within the District Nursing field. As a community nurse, you would be supported to develop into an autonomous practitioner; nursing patients with multiple complex health care needs in their own home.

Main duties of the job:

  • Community nursing is a specialised area of practice.
  • As a Community Staff Nurse, you will work within a locality team of District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners and Healthcare Assistants, to meet the needs of the complex patient in the community and avoid admissions to acute settings.
  • This includes medication administration, end of life care, managing the deteriorating patient, complex and simple wound care including negative pressure wound therapy and compression therapy, venepuncture, utilising advanced assessment skills and clinical nursing skills.
  • We nurse people with a variety of conditions in a holistic manner in their own home and in residential care settings.
  • Community nurses work in close partnership with social care and other primary health care professionals such as GPs, Community Therapies and specialised services such as Tissue Viability, Diabetes and Heart Failure.

  • EDUCATION, QUALIFICATIONS & TRAINING
  • Registered Nurse/Awaiting NMC PIN
  • Committed to own professional development to enhance practice
  • Post registration education and extended skills e.g. Intravenous drug administration, compression dressings.
  • Evidence of further continued professional development
  • Hold / be working towards an NMC approved mentoring qualification
  • SKILLS, ABILITIES & KNOWLEDGE
  • To be able to communicate effectively with patients, relatives, carers and other professionals.
  • To be able to care for patients without prejudice.
  • Able to work under pressure.
  • Self awareness, self motivated, enthusiastic.
  • Ability to manage own caseload.
  • Knowledge of the NMC Code of Professional Conduct.
  • Knowledge of the Data Protection Act.
  • Post registration experience working in an acute hospital setting.
  • EXPERIENCE
  • Demonstrate experience post or pre-registration placements in relevant clinical areas: Care of Elderly, Community, Orthopaedics, Neurology, Respiratory, Palliative Care, Long term conditions.
  • Ability to understand the role of a registered nurse.
  • Ability to work as part of a team and on own initiative.
  • Awareness of current professional nursing issues.
  • Demonstrates understanding of the importance of audit/quality.
  • Desirable but not essential knowledge of working with a single assessment, planning and the delivery of care.
  • Experience of electronic clinical systems and technology e.g. RIO.

SPECIAL CIRCUMSTANCES

Able to travel to other sites and off site as necessary.
