bthechange CIC is recruiting a Prison & Community Transition Coordinator (PCTC) to work across two prison sites and community settings in the South West. This role involves supporting both men and women in custody, ensuring a seamless transition from prison into the community.
The successful candidate will:
- Work inside prison at two sites (including weekly travel to Bristol & Gloucester areas)
- Support individuals at least 12 weeks prior to release
- Collect individuals on the day of release
- Provide a minimum of 20 weeks structured post-release support
- Hold and manage a caseload
- Deliver strength-based assessments and harm reduction interventions
- Develop personalised transition and recovery plans
- Coordinate closely with Housing, Drug & Alcohol services, Employment providers, Prison and Probation services
This is a relationship-based, frontline role requiring flexibility, resilience, and strong partnership working.
Travel Requirement
This role requires:
- Weekly travel to prison sites in the Bristol & Gloucester area
- Community-based work across the South West
- A full UK driving licence and access to a vehicle (essential)

Person Specification
Essential:
- Experience / knowledge of working within the criminal justice system
- Experience supporting vulnerable individuals, including substance use recovery
- Experience managing a caseload
- Strong communication and partnership-working skills
- Ability to work independently across multiple sites
- Full UK driving licence and access to a vehicle
Desirable:
- Experience working with both men and women in custody
- Trauma-informed practice experience
- Lived experience (welcomed and valued)
