The Successful post holder will work within Disability Day Care Services in the Southern Health & Social Care Trust, providing person centred nursing care and support to service users, their families and carers within Disability Day Care settings. The post holder will be expected to work flexibly across day care settings trust wide as required to meet the needs of the service. The post holder will work collaboratively as part of a multidisciplinary team, ensuring the delivery of responsive care by assessing, planning, implementing and evaluating care to meet assessed need within allocated resources.

Responsibilities

  • Ensure the provision of high quality care to service users in partnership with family and carers in accordance with assessed need.
  • Contribute to service development, audits and training.
  • Have responsibility for the oversight of clinical practice of Band 5 day care nurses.
  • Lead and provide professional supervision to day care nurses.
  • To co-operate and work effectively with other professionals in the multi-disciplinary team and community services to ensure continuity of care.

Skills and Qualifications

  • Currently a Registered Nurse Level 1 Learning Disability or Adult Nurse - on the Live NMC Register.
  • Minimum of 3 years post registration experience as a registered nurse.
