To provide a specialist nursing service to patients with heart failure across a seven-day heart failure service. To assist in the ongoing development of the heart failure service in a collaborative primary, community and secondary care service. You will also act as a resource for other health care workers by offering expert advice and education. You will assist in ongoing research and audit programmes within the heart failure service.
Responsibilities
- Manage patients with a confirmed diagnosis of heart failure within recognised and agreed international, national and regional heart failure guidelines.
- Be responsible for the direct delivery of clinical care and be competent to act.
- Undertake triage of referrals to the nurse led heart failure service, seeking advice from the heart failure advanced nurse practitioner (ANP) or consultant when appropriate.
- Undertake a holistic assessment (including health assessment/clinical examination) of patients’ needs encompassing physical, emotional, social and spiritual issues, including, the needs of the family and carers.
Skills and Qualifications
- 5 years post registration experience in a cardiology setting, 2 of which must be at Band 6 and above.
- Currently a Registered Nurse (Adult) on the live NMC Register.
- Degree in Nursing or other health related subject and evidence of a post Registration qualification in cardiology nursing.
