The post holder will:

  • Facilitate the provision of high quality, specialist care to patients with acute and chronic heart failure, assisting in the diagnosis, treatment and follow-up.
  • Work as an autonomous practitioner, to assess patients, determine appropriate management strategies and evaluate the effectiveness of interventions.
  • Interact with heart failure patients in a variety of settings: in-patient wards, out-patient clinics, ambulatory care centre and help-line calls.
  • Contribute to the evaluation and development of the specialist nursing service for heart failure patients based on patient feedback, audit, research and guidelines.
  • Work in collaboration with the multi-professional team to ensure excellence in patient care, acting as patient advocate and contributing to clinical decision making.
  • Undertake independent prescribing within the specialist nursing service for heart failure patients, whilst adhering to non-medical prescribing protocols and guidelines.
  • Act as a role model, demonstrating effective leadership and promoting excellence through a high standard of practice.
  • Contribute to the delivery of the organisation’s objectives.
  • Promote and monitor safe and effective practice.
  • Enhance the patient/client experience.

Responsibilities

  • Adhere to DHSSPS and NMC guidelines for practice requirements and standards, for example, safe handling, administration, storage & custody of medicinal products.
  • Adhere to Trust guidelines, policies and procedures.
  • Ensure that organisational goals are reflected in own and the nursing teams’ objectives and in the heart failure service.
  • Contribute to the development of the Trust’s policies and strategies, where appropriate.

Skills and Qualifications

  • Have 5 Years Post Registration Experience including 3 years’ recent experience in an Acute Cardiology setting.
  • Extensive experience of nursing patients living with heart failure.
  • Registered Nurse on Part 1 of the Live NMC Register.
  • Previous experience in the role of Heart Failure Nurse Specialist.
  • Experience or running nurse-led clinics.
  • Experience of managing a caseload of patients.
