Community Psychiatric Nurse for Physical Health Monitoring Band 6
The practitioner will work as a member of the core Physical Health Monitoring Team across the trust. They will work within a matrix organisation promoting effective inter-disciplinary and inter-agency relationships and acting as a link between clients, professionals and other services to ensure continuity of care. As a member of the physical health monitoring team, the post-holder will provide skilled assessments and provide evidenced based brief interventions to clients in relation to their physical health needs, liaising with relevant agencies and carers. Mental Health services are flexible and responsive requiring staff to work between the hours of 9am β 5pm Monday to Friday, the person appointed must be flexible in working over all three areas in Down, Lisburn and Ards Clinics as necessary. The successful applicant will also be required to work in any part of the South Eastern Health and Social Care Trust area within any mental health team or service as needs dictate.
Responsibilities
- To accept appropriate referrals, undertake assessment of individuals mental health and social needs and identify potential risks.

Skills and Qualifications
- Two years post-qualifying experience in Adult Mental Health; Registered Mental Health Nurse, live on the NMC Register.
