BS3 Community Development is looking for a thoughtful, analytical and people-focused Social Prescribing Link Worker to help strengthen and improve the impact of our social prescribing service through insight, learning and community connection.
Who we are… BS3 Community Development is an independent charity working across Southville and Bedminster. Our mission is to improve the health, wellbeing and quality of life for people who live and work in the BS3 postcode.
About the role… As a Social Prescribing Link Worker (Experience & Insight), you will focus on what happens after people are referred into social prescribing—helping us understand how individuals engage with support, what difference it makes to their lives, and how we can improve the service. Working closely with the Social Prescribing team, GP practices, community organisations and partner agencies, you will take a person-centred approach to capturing lived experience, following up with individuals, and gathering meaningful insight into their wellbeing journey. You will explore what helps people stay connected to community activities, identify barriers to engagement, and feed this learning back into the service to strengthen pathways, partnerships and outcomes.
What you’ll be doing…
- Proactively contact people who have completed or moved through the Social Prescribing service to explore how they are getting on, what activities or groups they have connected with, and what has helped or hindered sustained engagement.
- Use wellbeing- and outcome-focused conversations to understand changes over time, including confidence, social connection, self-care and use of community support.
- Capture qualitative and quantitative insight to build a stronger picture of the patient journey from primary care into VCSE activity.
- Identify patterns, successes and gaps in ongoing support, feeding learning back into the service and informing service improvements.
- Provide light-touch motivational support during follow-up where appropriate, helping people re-engage or adapt their support if circumstances have changed.
About you… We’re looking for someone who is:

- A strong listener, able to build trust and explore sensitive topics in a supportive way.
- Confident working independently and managing a varied workload.
- An excellent communicator with the ability to clearly capture and share insight.
- Comfortable working with a wide range of partners and stakeholders.
Experience in evaluation, community work, social prescribing, or health and wellbeing settings would be beneficial.

Contract: Fixed term for one year
