Job Purpose:
- Primarily field-based in Derry areas as required and Glendermott Road Derry when office-based.
- Arrange initial visits to service user homes and completion of Service User Agreements along with the completion of risk assessments for and with service users.
- Contribute to the ongoing assessment and review of need of each service user.
- Undertake and complete Quality Annual Monitoring visits with each service user.
- Check service user satisfaction by regular telephone contact.
- Deputise with the management of the day-to-day operations in the absence of the Service Manager.
- Carry out spot checks with staff and report outcomes to the service manager.
Minimum of 18 months paid experience supporting older people in a domiciliary/social care setting.
Flexible approach to working hours.
Full driving licence and access to a car.
Ability to provide on-call support to Care Workers outside of office hours.
